New Patients
If you are a new patient, please, where possible, complete any paperwork at home and bring it with you to the clinic. If you are unable to do this then hard copies will be available for you to complete.
Alternatively, please print out and complete the new patient registration form (found via the links below) and bring it to your assessment.
If you are seeing a physiotherapist in the clinic, please complete the Clinic New Patient Registration Form.
If you are being seen at home or in a care environment, please complete the Community New Patient Registration Form, a Covid-19 Screening and Consent Form, and Sharing of Information Form (if appropriate).
What to Expect at Your First Appointment
During your first appointment a detailed history of your condition, any past relevant history, will be taken. A physical assessment will then be performed and a treatment plan discussed.
So that we can properly examine your area of pain, please wear suitable clothing. Patients should wear shorts or loose-fitting tracksuit bottoms, for lower limb injuries, and a vest top for upper limbs.
You may be asked if you are happy to remove certain items of clothing to allow examination of your injury. You will always be asked for your consent to do this and may refuse at any time. You are more than welcome to be chaperoned by a friend or family member.
